Thank you for your interest in joining the Polish American Congress. Please read these instructions in their entirety before you proceed.

1. After reading the instructions, bring up the Membership Form by clicking "Go to the Form" below the instructions.

2. Fill out the form on the screen.

3. Print the form on your printer. It should be only one page. If it overflows, try resetting the font to a smaller size, or adjusting the margins of your page. Margins should be 0.75" or 0.5" on all four sides.

4. Sign the form where indicated.
